Home Health Surveyor – Starting Market Rate Salary $60,431.88

In this front-line field position, the surveyor conducts initial inspections, re-licensure inspections, follow up inspections, and investigations of complaints and incidents as a team member or individually, including complaints related to abuse, neglect or exploitation in Licensed-only Home and Community Support Services Agencies (HCSSA).  Investigates complaints and self-reported incidents regarding services provided by HCSSAs. The ideal candidate is a Licensed Vocational Nurse (LVN). The surveyor completes inspection/survey reports listing deficiencies/violations and prepares written reports supporting the actions and remedies recommended. Participates in surveys, inspections, and investigations with assigned team and accepts assignments as needed. Occasionally might need to testify at hearings or in court proceedings to provide information regarding policy/procedures, history, findings, or activities. Makes appropriate referrals to state agencies.

Essential Job Functions (EJFs):

Attends work on a regular and predictable schedule in accordance with agency leave policy and performs other duties as assigned. 

 

Conducts licensure inspections and investigations using applicable protocols, state regulations, and appropriate policies and procedures in an effective, thorough, and professional manner. 

 

Conducts surveys and follow-ups as scheduled by the Program Manager.

 

Conducts complaint/incident investigations, including for allegations for abuse, neglect, and exploitation.  

 

Accurately determines compliance with HCSSA state licensure regulations.

 

Completes survey reports detailing deficiencies, recommended remedies, and narratives to support regulatory findings.

 

Writes statements of violations in accordance with the Principles of Documentation.

 

Coordinates all aspects of surveys, investigations, and inspections.  

 

Visits agencies/facilities regulated by Long-Term Care Regulatory (LTCR) to verify correction of previously cited regulatory violations.

 

Performs quality assurance reviews of documents prepared by others, for example, publications, assessments, and reports.  

 

Occasionally might need to testify at hearings or in court proceedings to provide information regarding policy/procedures, history, findings, or activities. 

 

Knowledge, Skills and Abilities (KSAs):

 

Knowledge of Regulatory Services policies and procedures.  

Knowledge of relevant laws and rules related to long-term care facilities.  

Knowledge of or experience in long-term care regulatory services including surveys, investigations, and or enforcement.  

Ability to conduct complex on-site facility inspections for compliance determinations.  

Ability to compose professionally written correspondence and legal material. Ability to analyze documents.  

Ability to multi-task, prioritize, plan, organize and monitor work.  

 

Registrations, Licensure Requirements or Certifications:

 

LVN, Preferred

 

Initial Screening Criteria:

 

Minimum of one-year experience in institutional care, home health care, hospice care, or long-term care.  

Four-year degree from an accredited college or university, preferred.   

Experience and education may be substituted for one another on a year-by-year basis, up to four years.

Additional Information:

Flexibility in work hours is required for this position. The job requires 80% travel, including some statewide travel, with most travel during the day. However, some overnight and weekend travel is required, as well as occasional on-call duties for emergencies or high-priority situations in facilities. Must have reliable transportation.

Note: This position is able to work a compressed workweek of four 10-hour days.
