Geriatric Facilities Generalist Surveyor – Starting Market Rate Salary $56,535.00
Under the supervision of the Program Manager, this position participates as a team member or team coordinator conducting inspections, surveys, and investigations in Long Term Care facilities (Nursing Homes, Assisted Living Facilities, Day Activities and Health Services) to determine compliance with state and federal laws, regulations, and rules. Completes inspection/survey reports listing deficiencies/violations and prepares written reports supporting the action and remedies recommended. This position offers a competitive starting salary; excellent state benefits, including health insurance and retirement; and the opportunity to regularly travel to different facilities within a region and across the state – Please see the Additional Information section below for more detail on travel. In this front-line field position, a surveyor is mobile and flexible and performs minimal work at an office, though the role does require writing reports detailing any deficiencies/violations against a facility, remedies to correct those deficiencies, and narrative to support regulatory determinations. Essential Job Functions (EJFs):
Conducts certification surveys, licensure inspections, or investigations in facilities to determine compliance with federal and state regulations.
Completes inspection/survey reports listing deficiencies and prepares written reports supporting the actions and remedies recommended.
Visits facilities to verify correction of previously cited regulatory violations.
Conducts exit conferences with facility staff regarding findings and may assist in developing a plan to correct deficiencies.
Communicates on a complex level with others (internally or externally) to provide, exchange, or verify information, answer inquiries, address issues, or resolve problems or complaints.
Testifies at hearings or in court proceedings to provide information regarding policy/procedures, history, findings, or activities.
Performs quality assurance reviews of documents prepared by others, for example, publications, assessments, reports).
Completes referrals for conduct, as appropriate.
Knowledge, Skills and Abilities (KSAs):
Knowledge of program regulations and procedures, and survey techniques and principles. Knowledge of acceptable professional standards of care.
Skilled in operating personal computers and software.

Registrations, Licensure Requirements or Certifications:
Must successfully obtain Surveyor Minimum Qualification (SMQT) within the first year of employment.
Applicant may be a licensed nurse (LVN or RN), Social Worker (LMSW, LCSW), Registered Dietitian with valid, state-issued license.
Licensed Vocational Nurses that apply must be licensed by the State of Texas (or a state that recognizes reciprocity through the Nurse Licensure Compact).
Initial Screening Criteria:
A Bachelor’s degree from an accredited college or university in a human service field (including but not limited to sociology, education, social work, psychology, rehabilitation, counseling, humanities, allied health, behavioral science, early childhood intervention, and medical administration). A combination of work experience and education may substitute for a Bachelor’s degree on a year by year basis for up to four years.
Experience in quality assurance/compliance and oversight/auditing preferred.
Experience in conducting investigations related to quality outcomes.
Experience in conducting investigations of abuse, neglect, or exploitation in a regulatory program.
Additional Information:
Flexibility in work hours is required for this position. The job requires 80% travel, including statewide travel, overnight, and weekend travel. On-call duties for emergencies or high-priority situations in facilities or at providers may be required. Must have reliable transportation.
Note: This position is able to work a compressed workweek of four 10-hour days.
