Job Description:
This position will be assigned to Region 10 HHSC-CCSE Unit 17 comprised of approximately 11-12 Community Care Case Worker staff. Provides direct delivery and contract services to long term care services clients in assigned geographic area. Screen requests for people who are aging and those with disabilities who are applying for Long Term Care Services and supports that enable them to remain in the community. Work involves interviewing elderly consumers and consumers with disabilities, explaining., and referring consumers to the appropriate services. Processes Community Care Services & Eligibility (CCSE) referrals for unit assignment. Provides general and specific information about CCSE services to individuals and the public. Makes appropriate referrals to other program areas and to the community organizations. Generates change requests from consumers, family members, and/or contracted provider agencies. Provides specific information regarding Interest List services and status of placements. Work includes operating a computer.
Essential Job Functions:
Attend work on a regular and predictable schedule in accordance with agency leave policy. Answers the toll-free Intake Line for Region 1/10 CCSE. Be familiar with and follow all HHS policies and procedures relating to job performance and work rules. Accept and process telephone and fax requests for CCSE program services. Provides general and specific information about CCSE program services. Refers Intakes to CCSE staff. Refers individuals to other programs, agencies, and other Community organizations. Completes special assignments and/or reports. Operates a PC to document, update or inquire into case records. Communicates on a basic level with others (internally and externally) to provide, exchange or verify information on CCSE services. Answer inquiries, address issues, or resolve/report problems or complaints. Operates a PC to document, update or inquire into case records. Prepares reports and records to provide management information on program operations. Performs related work as assigned.

Knowledge Skills Abilities:
Knowledge of customer services principles and practices. Ability to interview individuals and document essential information. Knowledge of Gerontology, the field of disability, the dynamics of the aging process and the special problems of the aged and disabled. Experience communicating effectively with all types of people, in all types of situations. Ability to operate computer equipment. Knowledge of business English. Education which demonstrates the possession of knowledge, skills, and abilities for job performance. Ability to listen to clients, exploring and reflecting feelings and present relevant alternatives describing feasibility and consequences of choices. Ability to complete work within required timeframes. Ability to appraise living conditions including physical and emotional environment. Ability to appraise nature and seriousness of threat to an elderly or disabled person. Ability to establish priorities, according to the relative importance and deadlines, set goals and manage time effectively. Ability to communicate orally and in writing. Ability to establish rapport with clients. Ability to communicate with community members to request assistance for clients, promote support for the agency or coordinate resources and programs.
Initial Screening Criteria:
- Application indicates any current customer service experience interacting with individuals or customers in a retail or social service setting, assisting customers or explaining benefits or products or screening individuals for services either by telephone or in person.
- Application indicates applicants have attained at least a High School diploma or GED equivalent is required. Prefer attainment of at least Sixty (60) semester hours from an accredited college or university in the disciplines of Social Work, Psychology and/or Sociology.
- Application indicates experience in using general office equipment such as using a computer or software that may include Microsoft Office Products such as Outlook, Word, Excel, Power Point and SharePoint.
- Application indicates applicant has the experience to communicate effectively orally and in writing, both in English. Bilingual in English and Spanish preferred.
- Application indicates applicant can work a regular and predictable work schedule; specifically, the first shift, Monday -Friday, 8:00 am to 5:00 pm.
