The Admissions and Placement Coordinator works under the general supervision of the Assistant Director of Program Services. The Admissions and Placement Coordinator coordinates services for individuals at the Brenham State Supported Living Center. Serves as primary facility contact for State Office, Local Authorities, other state facilities, and the courts. Facilitates residential admission and community placement for the facility. Assists Individual Supports Teams in complying with internal and external policies and procedures related to admission and discharge. Effectively communicates with families and friends of person’s served to ensure compliance with internal/external policies and procedures.
Essential Job Functions (EJFs):
Attends work on a regular and predicable schedule in accordance with agency leave policy and performs other duties as assigned.
Coordinates facility admissions. Serves as facility's primary contact for external service request. Provides initial orientation of families and residents to facility services. 2. Works with State Office and other Admissions Coordinators to ensure the appropriate transfer of residents to and from the facility. Serves as facility coordinator for all interstate transfer requests. 3. Ensures appropriate commitment status for residents to include: coordination and completion of paperwork for submission to the court, coordinates with county court and county attorney's office, and coordinates resident, family, and IDT presence for hearings. 4. Coordinates data collection and completes reports on census, admissions, and discharges as requested by State Office. 5. Chairs the Admissions and Special Review Teams to determine appropriateness of services for admission requests and/or community returns. 6. Monitors facility vacancies to ensure appropriate utilization of bed space. 7. Coordinates the temporary consignment of state hospital psychiatric hospitalization. 8. Reviews and keeps current on the implementation of admission policies. Updates local policies as necessary. 9. Attends required meetings, participates in committee functions, and completes required reports and other assignment as provided by the ADOP. Other duties as assigned include but are not limited to actively participating and/or serving in a supporting role to meet the agency’s obligations for disaster response and/or recovery or Continuity of Operations (COOP) activation. Such participation may require an alternate shift pattern assignment and/or location.

Knowledge, Skills and Abilities (KSAs):
Knowledge of community-based services available to persons served. Ability to collect and analyze information to develop a community living plan. Knowledge of local, state, and federal laws and regulations relevant to program area with the ability to apply them to agency/facility policies and procedures and to convey them to others. Ability to develop and evaluate administrative policies and procedures, to prepare concise reports, to plan and facilitate meetings and communicate effectively orally and in writing. Excellent organizational and time management skills and meet deadlines. Demonstrated computer skills to include Word, Excel and Outlook. Experience in working with people who have developmental disabilities. Ability to demonstrate commitment to facility mission and workplace/service values. Ability to develop and provide training to staff, volunteers and persons served.
Registrations, Licensure Requirements or Certifications:
Must possess valid Texas driver’s license or obtain it no later than 90 days after hire date. Applicants with an out-of-state driver’s license must provide an original certified driving record from the state of driving licensure. Eligible driving record required based on HHSC Fleet Management policy.
Initial Screening Criteria:
Graduation from an accredited four-year university with major coursework in Psychology, Sociology, Special Education, Rehabilitation Counseling, Social Work, Communication, Business, or a Human Services field with at least one year of work experience in the related field, or at least five years’ experience in case management, information and referral, coordination of supports and services, transition/discharge planning, or project management.
At least one year of supervisory/management experience.
Experience working in community Immediate Care Facilities/Intellectual Disability (ICF/IID) and/or Home Community-Based Services (HCS) waiver programs is preferred.
Experience working with individuals with intellectual disabilities, and/or physical disabilities is preferred.
