The Neurologist for the State Supported Living Center (SSLC) performs highly advanced medical work, ensures and oversees the delivery of high quality, integrated medical services to individuals with intellectual disabilities served by HHSC State Supported Living Centers (SSLCs).   Diagnoses and treats individuals with intellectual and developmental disabilities who also have complex medical, psychiatric and/or behavioral health needs, with a specialty area of Neurology.  Conducts neurology clinics at SSLCs where a local Neurologist is not available on a permanent or on a temporary basis, both in person and via telemedicine, as approved. Provides and monitors medical services for assigned caseload. Provides guidance and recommendations to the state office discipline coordinators, SSLC facility administrators, and other clinical staff on issues related to neurological disorders. Provides staff education and training, direction and guidance in recognizing, responding to and managing neurological issues. Assists SSLCs in developing treatment plans with measurable goals, objectives and interventions to address neurological concerns.  Participates in meetings and committees such as mortality review committee, Quality Assurance/Quality Improvement (QA/QI) committee, SSLC morning medical meetings, polypharmacy meetings, and psychiatric clinics, as needed or assigned.  Identifies and communicates trends or issues and recommends solutions to the SSLC local leadership and State Office Medical Services Coordinator.  Completes required reports, medical documentation and required training.  Provide evaluations and advises medical providers on treatment to HHSC employees injured in course and scope of employment especially when related to Neurological issues. Participates with other members of the medical services department in initiatives to review and improve state-wide service delivery.   Shares on-call duty with other state-office medical staff if required. Works under the limited supervision with considerable latitude for the use of initiative and independent judgment. 

Essential Job Functions (EJFs):

Attends work on a regular basis and may be required to work a specific shift schedule or, at times, even a rotating schedule, extended shift and/or overtime in accordance with agency leave policy and performs other duties as assigned.

May serve in an on-call status.

Delivers, and consults in the delivery by others of, high quality medical services to individuals with intellectual disabilities served by SSLCs in accordance with current accepted medical practices.  Diagnoses and treats individuals with intellectual and developmental disabilities including individuals with epilepsy, movement disorders, multiple sclerosis, stroke, dementia, Parkinson’s disease, and other neurologic conditions. Conduct neurology clinics at SSLCs where a local Neurologist is not available on a permanent or on a temporary basis. Clinical care may be face to face with or without indirect contact, such as by telemedicine, as approved.

Provides guidance and recommendations to the state office and facility administrators, medical directors, primary care providers, psychiatrists, pharmacy, nursing, behavior health, habilitation therapy and other clinical staff on issues related to neurological disorders.  Provides direction and guidance to the local SSLC interdisciplinary teams (IDT) in medical issues and ensures health care plans related to neurological conditions are properly implemented. Assists SSLCs in developing treatment plans with measurable goals, objectives and interventions to address neurological concerns, and provides guidance to ensure medical treatment is properly integrated.  Coordinates with the local neurologist(s) for SSLCs when managing individuals with complicated neurological issues, as needed.

Provides staff education and training, direction and guidance in recognizing, responding to and managing neurological issues. As needed, participates in interdisciplinary support planning (ISP and ISP-A) meetings for individuals that reside at the State Supported Living Center and provides medical information to parents/guardians as appropriate.  Provides medical diagnosis and the review of the treatment plan at the Individual Service Plan (ISP). Evaluates the efficacy and progress of a patient's treatment plan and makes changes as indicated.

Participates in meetings such as mortality review committee, morning medical, polypharmacy, and psychiatric clinic, as needed or assigned.  Monitors medical trends for individual patients and for the SSLC as a whole and provides leadership to correct deficiencies.  Collaborates with psychiatry about individuals on dual use medications or when side effects of psychotropic medications may be suspected. Provides guidance on writing policies related to neuro-psychiatry issues and provide advice on improving Quality Assurance/Quality Improvement (QA/QI) processes related to neurological care.

Completes required reports, medical documentation and required training. Provides accurate and timely documentation related to the Neurology clinics conducted at facilities and cases referred for consultation.   As assigned, reviews documentation for individuals returning from the hospital, such as for uncontrolled seizures, to ensure continuity of care.  Follows up on any treatment or medication changes made while at the hospital. Stays current on assigned training and best practices in the area of neurological care for individuals with intellectual and development disabilities. 

Works collaboratively with other members of the medical services department to review and improve the quality of services provided. Assists in identifying trends and issues. Provides input into improvement plans for SSLC medical services, including improvements to policy and procedures, new forms of treatment and the need for new/advanced equipment. 

Provides evaluations and advise medical providers on treatment to HHSC employees injured in course and scope of employment especially when related to Neurological issues.

Other duties as assigned include but are not limited to actively participating and/or serving in a supporting role to meet the agency’s obligations for disaster response and/or recovery or Continuity of Operations (COOP) activation.  Such participation may require an alternate shift pattern assignment and/or location.

Knowledge, Skills and Abilities (KSAs):

Knowledge of methods and procedures in the field of general medicine and of the principle of the development, implementation, and documentation of individualized care and treatment plans.

Advanced specialty knowledge in the area of neurological disorders, such as epilepsy, movement disorders, multiple sclerosis, stroke, dementia, Parkinson’s disease, and other neurologic conditions.

Knowledge in working with psychiatric conditions.

Knowledge of medical jurisprudence.

Knowledge of, or ability to learn, ICF-IID standards, certification requirements for the Council on Quality and Leadership, and HHSC policies and procedures.

Skill in the care and treatment of patients and in the use of medical diagnostic and treatment tools and equipment.

Skills in diagnosing and treating neurological conditions.

Ability to communicate with nurses, direct care staff and interdisciplinary team members, and to work effectively as in interdisciplinary team members.

Ability to communicate effectively orally and in writing, and to work in harmony with individuals, employees, peers, family members, and medical/non-medical professions in the community.

Ability to evaluate patients and implement an effective course of treatment in coordination with other interdisciplinary personnel.

Ability to maintain required continuing medical education hours.

Registrations, Licensure Requirements or Certifications:

Must be licensed as a medical doctor by the State of Texas.

Must have current registration certificates from the Drug Enforcement Agency (DEA).

Must possess a National Provider Identifier (NPI#) as defined by Medicare Part B and enroll as a Medicaid and Medicare Part B provider.

Initial Screening Criteria:

Graduation from an accredited college of medicine/Osteopathic medicine program with a degree as a medical doctor/osteopathic physician. 

Boad certification in Neurology (By American board of Psychiatry and Neurology-ABPN) is preferred. Board eligible candidates with residency training will be considered.

Clinical experience in the area of neurology.

Experience leading, supervising or managing the work of others, preferred.

Flexibility in work hours may be required for this position. The position may be required to work overtime and/or extended hours. Must be willing to work weekends and holidays if needed.
